Once you finish filling in the electronic form, print out both sides of the form on the same A4 paper, sign your name and get it stamped by an authorized person before submitting it to the bank with all the other related documents.
e-Form is best opened with Adobe Acrobat 7.0 to get the benefit of the software features.
