- Contact the hospital in the AIA network
- Search or inquire about the network hospital list from AIA’s Website or Call Center
- Download form or receive form via e-mail
- Send the Pre-Authorization Form (PAF) to AIA
The Insured is required to
- Request your doctor to assess and fill the amount of medical expenses in the PAF
- Make an appointment for the treatment
- The Insured acknowledges and agrees to comply with the terms and conditions
- Sign the PAF
- AIA considers the request and approves the service based on the eligible benefits and coverage terms of the Insured* (submit the form at least 7 working days prior to the service).
- AIA issues the Letter of Guarantee (LOG).
- Admission to the hospital
- Dischargement from hospital (additional expenses may be incurred after hospitalization**)
*If any incurred medical expenses are beyond the amount of benefits under the Be Together Infinite Health or AIA Infinite Care Rider, such expenses will be the responsibility of the Insured.
**The Insured must pay the medical expenses that exceed the amount as specified in the Letter of Guarantee (LOG) and pre-approved by AIA. In this regard, the Insured can make an additional claim for the exceeding expenses only if such expenses are under the policy's coverage.
