*If the medical check-up result is preferred per the Company’s rules, the prospect policyholder will be entitled to a preferred rate discount. Please note that the preferred rate discount varies between the age of 18 to 75 and depends on your gender.

**The basic premium is tax-deductible up to 100,000 baht, according to the Notification of the Director-General of the Revenue Department on Income Tax (No. 172).

Benefits

1. If death occurs during the policy period

Receive the following benefit, whichever amount is greater:
1. 100% of the sum assured
2. Cash surrender value
3. Accumulated basic premium the Company has received. In the case that sum assured is decreased while the policy is in force, premiums received prior to the decrease in sum assured will be reduced in the same proportion

If the Insured dies during the policy year, the Company will calculate the premiums based on a full year premium.

2. If no death occurs during the policy period

Receive the following benefit, whichever amount is greater:
1. 100% of the sum assured
2. Cash surrender value
3. Accumulated basic premium the Company has received. In the case that sum assured is decreased while the policy is in force, premiums received prior to the decrease in sum assured will be reduced in the same proportion

Accidental Death Benefit (ADB) Rider

Accidental death

If the Insured is injured due to an accident and dies from the injury sustained in that accident within 180 days from the date the accident occurs, the Insured will receive an accidental death benefit equal to 100% of the sum assured from the rider.

Terminal Illness Benefit (TI) Rider

Terminal illness

If the Insured is first diagnosed and/or confirmed by a specialized medical practitioner that their sickness or injury will place the Insured in a terminal illness condition, the Insured will receive the following:
1. Terminal illness benefit
•  Receive a terminal illness benefit corresponding to the death benefit of a basic plan to which this rider is attached. The sum assured of this rider will be paid in proportion to the sum assured of basic plan.
  If the terminal illness benefit is paid out, the sum assured of the life insurance policy will be reduced by the amount of the sum assured of this rider which will also reduce of benefits in the life insurance policy.

Before the benefit is paid out, the company has the right to deduct any outstanding liabilities (if any).

2. Waiver of premium benefit
The Company will waive the basic premium if the company has paid the terminal illness benefit as above.

Other riders attached to this policy (if any) will still be in force as long as premium payments on these riders continue.


Note: Terminal illness is a diagnosis result or major injury that cannot be cured and consists of all the following conditions:
  1. An illness condition diagnosed by a medical practitioner with no specific treatment plan for its condition.
  2. A treatment plan that can only reduce the pain or symptoms of the Insured’s last life stage.
  3. The Insured is diagnosed with having last 12 months to live from the date of diagnosis by a specialized medical practitioner.

    100% of the sum assured of Be Together Legacy+ Life Insurance

    90% of the sum assured of Be Together Legacy+ Life Insurance

    50,000,000 baht for the Insured age from 60-75 years


  • Payment mode: Annually / Semi–annually / Quarterly / Monthly
    (If the premium is paid monthly, the first payment will include an advanced payment of two months.)

  • Semi-annually = Annual premium rate x 0.51

Age

Tobacco Use

1. No tobacco or nicotine products usage for last two years. Occasional cigar smoker may qualify if he/she smokes fewer than two cigars per month
2. Micro urinalysis is negative of nicotine

Alcohol and Substance Usage

No history of alcohol or drug abuse or treatment within the last 10 years

Blood Pressure

Age below 59 years old: Blood pressure is up to 150/90
Age above 60 years old: Blood pressure is up to 155/90

The Company will consider blood pressure results from an average of 6 months.

The above conditions will be applied to the prospect who is under treatment or nontreatment for high blood pressure.

Family History

1. For the prospect aged between 18 to 55, both parents must not have passed away before the age of 55 from the following diseases:
   
•  Coronary Artery Disease
   •  Colorectal Cancer
   •  Lung Cancer
   •  Melanoma Cancer

2. For the female prospect aged between 18 to 55, the mother must not have passed away before the age of 55 from the following diseases:
   •  Breast Cancer
   •  Ovarian Cancer

3. For the male prospect aged between 18 to 55, the father must not have passed away before the age of 55 from the following disease:
   •  Prostate Cancer

Personal Medical History without a Current Rateable Impairment

No history of
   
•  Diabetes
   •  Cardiovascular Disease
   •  Cerebrovascular Disease
   •  Cancer

The prospect who has had any kind of tumours which have been removed will be considered a standard rate right away. However the Company may consider offering the prospect a preferred rate on a case by case basis.

Body Mass Index (BMI)

Body Mass Index (BMI), Cholesterol and Cholesterol to HDL Ratio are to follow calculations and levels chart which depend on the underwriting criteria of the Company.

The above conditions will be applied to the prospect who is under treatment or nontreatment for high cholesterol.
